Robotic versus laparoscopic distal pancreatectomy: a propensity-matched analysis of 412 patients
In this propensity-matched cohort of 412 patients, robotic distal pancreatectomy was associated with significantly lower conversion rates (3.2% vs 12.1%, p<0.001) and shorter length of stay (4.2 vs 5.8 days, p=0.02) compared to laparoscopic approach. Operative time was longer in the robotic group (186 vs 142 min) but 90-day complication rates were comparable (28% vs 31%). These findings support robotic distal pancreatectomy as a safe alternative with potential advantages in complex anatomy.
